The Region 5 HMCC is the official name for the HMCC serving southeastern Massachusetts and the Cape and Islands, which covers the counties of Barnstable, Bristol, Plymouth, Duke's and Nantucket. In addition, the Region 5 HMCC also includes the Towns of Avon, Foxborough, Holbrook, Plainville, Randolph and Stoughton housed in Norfolk County.MDPH OPEM has designated 6 core disciplines essential to reprsent, and function as the HMCC in each region and to fulfill and coordinate regional health and medical emergency planning initiatives, Including:Community Health Centers & large Ambulatory Care PracticesEmergency Medical Services (municipal and private)Hospitals and Hospital Systems of CareLong Term Care facilitiesPublic Health Departments/Boards of HealthEmergency ManagementWhile these 6 core disciplines are required for the development and directives of the HMCC, any and all emergency  management partners, (dialysis centers, home health agencies, pharmacies, behavioral health, etc.) are always welcome to become members and attend Stakeholder Meetings, trainings and events, receive regional information and planning resoures, and may have a larger and more formal role with regional planning efforts in future years of the Region 5 HMCC development.
