Missouri Coalition Celebrating Care Continuum Change (MC5)

Formed in 2005 by Missouri's most influential senior health organizations, MC5 is a non-profit organization dedicated to reinventing the continuum of adult care in favor of a more person-centered approach. MC5 has 14 regions providing over 70 educational seminars throughout the year to educate adult care professionals and the public about the philosophy of person-centered care. The mission of MC5 is to promote and support ongoing collaboration within the adult care continuum to create environments where the dignity and value of each individual who lives or works there is respected and celebrated.
