Country to Coast QLD is a not-for-profit organisation and we deliver the Primary Health Network (PHN) program on behalf of the Commonwealth Department of Health. Our Vision is to support our region to live their healthiest lives and our Mission is to work with our community to deliver leading primary health.What we do:Coordinate primary health services to meet the needs of our communities, particularly those with high health needsImprove the patient journey between primary and acute careSupport general practiceImprove access to health services, especially for people in remote areasEase pressure on hospitals.
