Navajo Nation DALTCS

Navajo Nation Family Caregiver Support Program is intended to provide a multi-faceted system of support services for the informal, unpaid, family caregiver for their grandparents or older individuals who are family caregivers. For caregivers who informally provide care to older individuals, the program will help sustain their efforts to care for older individuals who have a chronic illness or disability and will promote the ability of older individuals to remain in their homes and local communities for as long as possible before considering residential care facilities. Support program services includes: Information to caregivers about available services, Assistance to caregivers in gaining access to services and resources, Counseling on caregiver self-care, support groups, training, Respite Care to enable the caregivers to be temporarily relieved from their caregiving responsibilities, and the Supplemental services required to follow-up on respite care.
