The Transitions Life Center Community Navigator, in collaboration with the Marion County Hospital District, serves as a vital resource for families and caregivers of individuals with intellectual and developmental disabilities (I/DD). This program acts as a bridge, connecting these families to essential social, educational, and health services, as well as specialized programs designed to enhance their quality of life. Best of all, this support is provided free of charge to all families caring for someone with I/DD.
